Hypoxic pulmonary hypertension: the paradigm is changing

چکیده
Sustained hypoxia caused by migration of native sea-level dwellers to high altitude or by chronic lung disease leads to the development of increased pulmonary vascular resistance and pulmonary hypertension, a response to hypoxia that is unique to the pulmonary circulation. In susceptible individuals at high altitude (subacute and chronic mountain sickness) and in lung disease, the resultant pulmonary hypertension causes significant disability and reduced life expectancy. For these reasons, investigation of the underlying mechanisms is an area of intense research activity. Classically, the elevation in pulmonary vascular resistance in chronic hypoxic pulmonary hypertension has been

Hypoxic pulmonary vasoconstriction in COPD-associated pulmonary hypertension: been there, done that?
To many of us involved in teaching pulmonary physiology and gas exchange to medical students, hypoxic pulmonary vasoconstriction (HPV) is a favourite topic. The unique property of small pulmonary arteries to constrict in response to a reduced alveolar oxygen tension allows us to cover not only the complexities of ventilation perfusion matching, but also to dive into the more exciting “physiolog...
متن کاملSustained Hypoxic Pulmonary Vasoconstriction in the Isolated Perfused Rat Lung: Effect of α1-adrenergic Receptor Agonist
Background: Alveolar hypoxia induces monophasic pulmonary vasoconstriction in vivo, biphasic vasoconstriction in the isolated pulmonary artery, and controversial responses in the isolated perfused lung. Pulmonary vascular responses to sustained alveolar hypoxia have not been addressed in the isolated perfused rat lung. In this study, we investigated the effect of sustained hypoxic ventilation o...
متن کاملThe Interaction between Trolox and 4,4’-diisothiocyanatostilbene-2,2’-disulfonic Acid on Hypoxic Pulmonary Vasoconstriction in the Isolated Rabbit Lung
Background: The mechanism of hypoxic pulmonary vasoconstriction (HPV) is still debatable. It has been proposed that reactive oxygen species (ROS) might be involved in HPV. However, there is no special transporter for superoxide anion in the cell membrane and it may release from the cells via anion exchanger. Therefore, the aim of this study was to investigate the interaction of ROS and anion ex...
متن کاملHypercapnia attenuates hypoxic pulmonary hypertension by inhibiting lung radical injury.
Chronic lung hypoxia results in hypoxic pulmonary hypertension. Concomitant chronic hypercapnia partly inhibits the effect of hypoxia on pulmonary vasculature. Adult male rats exposed to 3 weeks hypoxia (Fi(02)=0.1) combined with hypercapnia (Fi(C02)=0.04-0.05) had lower pulmonary arterial blood pressure, increased weight of the right heart ventricle, and less pronounced structural remodeling o...
متن کاملJAP-01337-2004 R.1 Thromboxane inhibition reduces an early stage of chronic hypoxia-induced pulmonary hypertension in piglets
The pulmonary vasoconstrictor, thromboxane, may contribute to the development of pulmonary hypertension. Our objective was to determine if a combined thromboxane synthase inhibitor/receptor antagonist, terbogrel, prevents pulmonary hypertension and the development of aberrant pulmonary arterial responses in newborn piglets exposed to 3 days hypoxia. Piglets were maintained in room air (control)...
